After we last see him in "Alexandria, Why?" Egyptian filmmaker Yehia Mourad is in his thirties, and successful in his work, he has grown distant from his wife and children and suffers a symbolic blockage of the heart while shooting the final scenes of his latest film. After being flown to England for evaluation, it's determined that Yehia must undergo emergency surgery. Fact and fiction blend seamlessly—with healthy doses of cleverly absurdist fantasy—as the film explores the various personalities and forces that have made Yehia (and Youssef Chahine) the man he has become.

(Zainab) escapes from prison to meet her children, and searches for evidence of her innocence, which is a letter hidden by her brother (Ahmed).

Ahmed gets acquainted with Laila and relates to her, and she agrees to marry Omar without knowing that he is Ahmad's brother. Ahmed informs Laila of a wedding without knowing that he is Omar. Layla thinks he has given up on her and decides to marry Ahmed and Omar lives with them.

Mansour Nassab deceives the new arrivals to Cairo, falls on his way to the shy young man Sharif, and claims to him that he is rich and is managing a place for him to stay in, so he resorts to a hotel owned by Tahiya, who lives in it with her daughter Sawsan, and in the hotel Sharif gets to know the uncle of the old man who He sees that this young man is in need of care and a guide for him, and Mansour agrees with Sharif to kill Sawsan's father and Tahiyya's husband in search of great wealth, provided that they share the wealth between them
